If you are a new patient to our office, the attached file contains our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. My first time as a patient at comprehensive family dentistry and my experience was exceptional. The exam was very thorough especially with the high tech devices used to evaluate. The staff and doctor were very friendly and knowledgeable. The best part was the cleaning! Normally I dread this part but the hygienist made me feel Comfortable and explained each step. Will definitely recommend to my friends and family!
